Procedure Specific Consent Forms
To download the full contents of the Consent Form compact disc, please click the CD image to the right.
This will download a WinZip executable file called ConsentForms.exe.
To install:
• Double-click the downloaded ConsentForms.exe file.
• Click the UNZIP button that appears in the WinZip dialogue box -this will place a folder on your PC desktop called Consent Forms
• Open this new folder and run the file medical.exe by double-clicking its icon.
